TVPD has joined Project Safeguard, a community program designed to help provide law enforcement with quick access to important information regarding those with autism, dementia or other disabilities.

If you need assistance completing this form, you may come in person to our TVPD station, 2600 W. Taylorsville Blvd., during regular business hours. The information provided will be kept within TVPD and Salt Lake Valley police dispatch systems (VECC). You must be an adult family member or guardian to provide the information. Additionally, you must submit an updated form yearly.
